Thank you for choosing Texas Orthopedics for your orthopedic care. Once you have made your initial appointment, we encourage you to download and complete the patient history and intake form prior to your first visit and bring it with you. If the patient is a minor that needs to be seen without a parent or guardian, the “Authorization to Treat a Minor” form will need to be completed in order for us to treat a minor.

Medical Records Release

If you require a copy of your medical record or require the completion of a Disability/FMLA Form, please complete the associated form (links to forms provided below). Please be advised that requests for medical records and requests for disability form completion require separate authorization forms to be completed. Further information relating to process and fees for requests can be found on the second page of the forms. For any additional questions, please contact or Release of Information department at (512) 439-1000, Option ‘2’, then Option ‘1’. If you require expedited copies of records for Continuation of Care purposes, your medical care provider may request records directly from our offices via phone at (512) 439-1000, Option ‘2’, then Option ‘1’.

Referrals

Orthopedic Office Visit If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.

Dr. Logan sees patients at the Cedar Park location on Monday morning, Tuesday afternoon, and all day Friday. He sees patients at the Round Rock location on Wednesday and Thursday mornings.

In the event of a life-threatening medical emergency, call medical emergency services at 911 immediately. DO NOT WAIT! Inform the emergency room staff that you are a patient of Texas Orthopedics, and they will contact us.

If you have an urgent medical need and need to talk to Dr. Logan after our regular hours, please call (512) 323-5465. A doctor is on-call 24 hours a day. Your call will be returned promptly.
